PCN Services 

GP Assistants (GPAs)

​

At Wigan Central Primary Care Network (PCN), our GP Assistants (GPAs) play a key role in supporting practices to deliver high-quality, patient-centred care.

Working alongside GPs, nurses and the wider multidisciplinary team, GPAs help manage both administrative and clinical support tasks, enabling clinicians to spend more time caring for patients.

 

What do GP Assistants do?

 

GPAs support practices by:

​

  • Carrying out NHS Health checks

  • Processing clinical correspondence and patient records

  • Managing referrals and follow-up actions

  • Preparing information for clinics and patient reviews

  • Supporting chronic disease monitoring programmes

  • Carrying out routine observations and other delegated clinical tasks, where appropriately trained

  • Helping patients navigate services and access the care they need

How do GPAs benefit patients?

​

By taking on a range of non-complex administrative and clinical duties, GPAs help improve practice efficiency, increase appointment capacity, and support a smoother patient experience.

Community Link Workers​

 

At Wigan Central PCN, our Community Link Workers help people access the practical, emotional and social support they need to improve their health and wellbeing. We understand that many factors affect health beyond medical conditions. Issues such as loneliness, housing concerns, financial pressures, unemployment, caring responsibilities, low confidence, or social isolation can have a significant impact on both physical and mental wellbeing.

 

Community Link Workers work alongside GP practices and local organisations to connect people with services, groups and activities within their local community. Through personalised support, they help individuals identify what matters most to them and take positive steps towards achieving their goals.​

 

What is Social Prescribing?​

 

Social prescribing is a way of supporting people to access non-medical services and activities that can improve their health and quality of life. A Community Link Worker will take time to listen, understand your situation, and work with you to find practical solutions and opportunities that suit your needs and interests.​

 

This might include support with:​

 

  • Improving mental health and emotional well-being

  • Tackling loneliness and social isolation

  • Building confidence and self-esteem

  • Accessing benefits, debt or housing advice

  • Finding volunteering or employment support

  • Connecting with local community groups

  • Increasing physical activity and healthy lifestyles

  • Supporting carers

  • Accessing bereavement support

  • Developing new skills and interests​

How Community Link Workers Can Help ​Our Community

 

Link Workers provide person-centred support, empowering people to take greater control of their health and wellbeing.​

 

They can:

  • Connect you with activities that improve wellbeing

  • Work with partner organisations across Wigan to ensure you receive the right support

Who Can Access the Service?​

 

The service is available to patients registered with a GP practice within Wigan Central PCN who may benefit from additional social, practical or emotional support.​

 

You may be referred by:​

 

  • Your GP Practice

  • Nurse or Healthcare professionals

  • Social care professional

  • In some cases, self-referral may also be available through participating practices.​

What to Expect​:

 

Following referral, a Community Link Worker will arrange a conversation with you to discuss your needs, strengths and goals. Together, you will develop a personalised plan and identify local services, groups or activities that may benefit you. The support offered is flexible and focused on helping you achieve sustainable improvements in your health and wellbeing.​

The PCN Care Home Service

 

This service provides dedicated clinical care and support across Wigan Central PCN's allocated care homes. Our specialist multidisciplinary team, comprising an Advanced Clinical Practitioner, Paramedics, a Pharmacy Technician, and a Care Coordinator, takes a proactive approach to delivering high-quality, person-centred care.

 

Working closely with care home staff, residents, families and wider health and care partners, the team ensures residents receive the right care at the right time. Through regular multidisciplinary home rounds, comprehensive assessments, personalised care planning, structured medication reviews and vaccination checks, we support the health, wellbeing, and independence of care home residents.

 

The service is aligned with the Enhanced Health in Care Homes Framework, promoting effective communication, shared care planning, improved clinical outcomes and a coordinated approach to healthcare delivery across care home settings.

CYP Social Prescribing Service (age 5 – 25)

 

At the heart of social prescribing is the belief that every child and young person is unique. By working collaboratively and focusing on individual strengths, aspirations, and interests, the social prescribing team enable young people take control of their own wellbeing to achieve positive, sustainable changes in their lives.

 

The Children's and Young Persons (CYP) Social Prescriber team supports children, young people, and their families to improve their health, wellbeing, and life opportunities by connecting them with community-based activities, services, and resources.

 

The team provide one-to-one and group support utilising a person-centred approach, helping individuals to identify their strengths, goals, and needs, addressing the wider social, emotional, environmental, and practical factors that impact wellbeing.

Mental Health Support (Adults)

 

Taking the first step to seek support can be difficult, but you do not have to face things alone.

 

Our dedicated Mental Health Practitioner offers a confidential, supportive conversation to help you explore any mental health challenges you may be experiencing and discuss what matters most to you. Together, you will identify your needs and goals, receive practical advice and emotional support, and be guided towards the most appropriate services and resources.

 

Whether you are struggling with stress, anxiety, low mood, or other mental health concerns, we are here to help you access the right support at the right time.

Care Coordinators

 

Managing your health and wellbeing can sometimes feel overwhelming, particularly if you are attending multiple appointments or receiving support from different services. Understanding who does what, what help is available, and how to access the right support at the right time can be challenging.

 

A Care Coordinator can help make this process simpler. They work alongside you to understand your individual needs, helping you navigate health, social care and community services. By taking the time to listen and understand your circumstances, they can connect you with the most appropriate support, ensuring you receive coordinated care that works for you.

 

Care Coordinators can help you:

 

  • Understand and manage your care journey across different health and social care services.

  • Access local services and community resources that support your health and wellbeing.

  • Coordinate appointments and referrals where appropriate, helping to reduce confusion and duplication.

  • Ensure your needs, preferences and goals are recognised when planning your care.

  • Support you in making informed decisions about your health and the services available to you.

  • Help you access practical, emotional and social support that may improve your overall wellbeing.

 

Whether you are living with a long-term condition, recovering from illness, managing complex health needs, or simply need help understanding the support available, a Care Coordinator can act as a single point of contact to guide you through the system and help you access the care and services that are right for you.

 

Patients registered with a GP practice within Wigan Central PCN may be referred to a Care Coordinator by a member of the healthcare team when additional support is needed to help manage their health and wellbeing. Their aim is to provide personalised, joined-up care that empowers people to live healthier, more independent lives.
